Week 1 Contact HR Department at Clinic or Health Center to complete orientation and establish schedule with Preceptor. Complete on-site training. Complete Module 1 Partner with clinic or health center and community agencies to identify community need.
Week 2 Complete on-site training. Apply learning of Primary Care and Clinical Skills Complete Module 2 Conduct further research and needs assessment of health issue.
Week 3 Complete on-site training. Apply learning of Underserved and Community Health Centers Complete Module 3 Develop community project, such as brochure, report, presentation).
Week 4 Complete on-site training. Apply learning of QI, Cultural Competency and Patient-Centeredness Complete Module 4 Review community project with mentor and complete report.
Week 5 Complete remaining on-site training. Turn-in Time Sheet to Donna Treleven to receive your student stipend. Complete Post Evaluation Survey Disseminate Community Service Project and Submit Project Report
