Pre-Deployment Medical Evaluation (immunizations,
travel visit and kit, prophylaxis or
self-treatment if indicated, PPE)
Event Specific Medical Coordination Meeting

Clinical threshold as determined by OHC prior to
deployment into effected area. (For Avian flu
Fever gt100.4 and respiratory symptoms and
current or recent work with avian/pandemic
influenza.

Team member meets clinical threshold
1) Deployed team leader or 2) CDC event team
leader in Atlanta. The event team leader can
be reached through the DEOC 24/7 at 770.488.7100.

Team member notifies team leader
Outside agency POC contacts CDC DEOC duty officer
for potential evacuation of non-CDC infectious
patient 770-488-7100

Team leader contacts CDC DEOC duty officer

Try to contact OHC physician while on the phone
with the individual or team leader. 404.639.4444
DEOC duty officer calls CDC Occupational Health
Clinic (OHC) or the physician on-call

Notification Process takes place
Discuss clinical condition, mitigating factors,
requirement for evacuation, alternatives, etc.
OHC physician contacts team leader, the CDC
infectious disease liaison with Emory, and the
OHS Medical Officer to discuss case
DEOC duty officer contacts the Incident Support
Team Leader to put on alert

OHC physician contacts the Logistics Support Team
Lead to discuss options
DEO Logistics Support Team Leader contacts the
following people to put on standby OGH Duty
Officer NCID EC Event Communications POC

OHC clinic physician contacts the Logistics
Support Team Lead, who in-turn, notifies the
other team members. Can be done via telecon with
EOC duty officer facilitating a bridge. Should
include the Team Lead if available.
Recommend action
OHS Director contacts CDC Director with
recommendation. Director Decision

Team may contact CDC Director via telecon with
EOC duty officer providing a bridge.
Do not evacuate

Monitor the situation
The NCID EC will make contact with CDC
staff that have a need to know the situation.
He/She maintains lists of those that require
notification and is responsible for making the
notifications.
Evacuate
Evacuation cell 24/7 until mission
complete Single POC for the CDC during the
evacuation. Responsible for planning and
execution Staffed as required by operations,
clinical and support staff.

Activate the EOC medical evacuation cell at
404-639-5122
Initiate MEDEVAC procedures

If monitoring is required, will be done by
occupational health clinic personnel. If
condition changes, assemble team again for
re-evaluation and recommendation.
