preidentified label. This includes students who
took the GEE February Seniors Only Retest.
If a student took no part of a test and did not

If a student does not receive preidentified
labels and must test, the test administrator must
affix a nonpreidentified label to each of the
students answer documents. The test
administrator must hand code student
identification information on each answer
document according to directions in the Test
Administration Manual. The fields must be coded
exactly the same on all answer documents for a
student. If a student receives a single
preidentified label but needs to test using two
or three answer documents, the answer document
with the nonpreidentified label must be hand
coded to match the information on the
preidentified label exactly.
22
Applying Bar-code Labels to Answer Documents
Answer documents that are to be scored should
have a preidentified or nonpreidentified label
applied here.
The District Test Coordinator must apply,
nonpreidentified labels to answer documents for
approved home study program or LEAP nonpublic
school students who did not receive preidentified
labels.
23
Approved Home Study Program and LEAP Nonpublic
School Student Labels
  • The District Test Coordinator will distribute to
    appropriate summer testing sites preidentified
labels for approved home study program and LEAP
    nonpublic school students.
  • If approved home study program or LEAP nonpublic
school students do not receive preidentified
labels, the test administrator must hand code the
answer documents but must not affix labels from
the summer testing site to these answer
documents. The District Test Coordinator will
affix nonpreidentified labels to these answer
    documents after testing.
  • School Test Coordinators will place all answer
documents for approved home study program and
LEAP nonpublic school students in the provided
plastic bags and give them directly to the
    District Test Coordinator.
  • The District Test Coordinator must remove these
answer documents from the bag, affix
nonpreidentified labels with site code 998 or 999
to those without preidentified labels, and return
these answer documents to DRC in pickup 2.
  • See page 14 in the Test Coordinators Manual for
returning test materials for approved home study
program and LEAP nonpublic school students in
    pickup 2.

Other Labels

Test Not Taken Labels(affixed by School Test
Coordinator)
After all makeup testing is complete, the School
Test Coordinator must affix yellow Test Not Taken
labels to unused answer documents that have a
bar-code label and/or hand-coded student
information. Each unused answer document with a
bar-code label or hand-coded student information
must be inspected for student responses before
affixing the labels. Test Not Taken labels
should be applied over preidentified or
nonpreidentified bar-code labels. Unused answer
documents without bar-code labels or hand-coded
student information do not need a label. The
School Test Coordinator must indicate on the
Testing Site Security Checklist which unused
answer documents received Test Not Taken labels.
Unused answer documents without bar-code labels
or hand coded student information do not need a
label.

Unscorable Document Labels(affixed by the
District Test Coordinator)
UNSCORABLE DOCUMENT LABEL (BLUE)
Unscorable document labels must be used when an
answer document is soiled or damaged or when a
student took a test on an answer document with a
preidentified label for another student. The
damaged document must not be thrown away, and the
test administrator must contact the School Test
Coordinator (STC) immediately if an answer
document should be designated unscorable. The STC
then must contact the District Test Coordinator
(DTC) immediately. The DTC will provide
instructions for transferring student responses
to a scorable answer document. The new scorable
answer document must be returned by the school
with all other scorable answer documents. The
DTC will affix a blue Unscorable Document label
to the damaged answer document and return it with
the unused district overage answer documents in
pickup 3. Documentation regarding the incident
must be kept on file at the district office for
one year.
