attendance record periodically to determine if
absence indicates a work performance problem.
- Supervisor speaks with employee to determine if
employee has - A potential FMLA Qualifying Condition
- A potential ADA covered request (stated in the
form of a specific need)

1. Supervisor contacts HR Officer HR sends
employee FMLA packet with preliminary designation
as FMLA Leave
Supervisor informs employee that pattern of
absence is problematic discuss with HR Rep next
steps including possible referral to NYU
resources and discipline, if appropriate
2. Supervisor contacts HR HR consults with
Employee Relations to determine if ADA
accommodations are available or necessary
