SCIENCE LABORATORY USAGE OF SHS

• PURPOSE
• The purpose of these guidelines and procedures is
  to provide a set of standards in reserving any
  laboratory room to conduct classes, investigatory
  and research projects, remedial or special
  activities of the SHS Department in the
  Intramuros and Makati campuses. This includes the
  guidelines in lending of apparatus, equipment and
  issuance of chemicals.

• GUIDELINES FOR THE USE OF CHEMISTRY LABORATORIES
  AND FACILITIES
• The SHS faculty member shall submit the
  accomplished Laboratory Reservation Form for
  Faculty at least 2-3 days to ILMO before the
  scheduled laboratory activity
• The assigned laboratory room shall only be used
  on the reserved day/s based on the official class
  schedule and it the professor is already present.
• The accountability of the laboratory facilities
  shall be lodged to the faculty member assigned to
  the laboratory class and protected from damage,
  vandalism and misuse.
• The laboratory Technician shall require all
  students and faculty member to sign the
  Laboratory Safety Rules and Regulations and
  Conforme form.

• USE OF MANDATORY PERSONAL PROTECTIVE EQUIPMENT
  (PPE)
• All laboratory instructors, Laboratory Technician
  and students shall not be allowed to perform
  laboratory activities without their PPE. The
  following are the mandatory safety gears to be
  worn at all times during the laboratory
  classes/activities
• Laboratory Gown
• Neoprene/Nitrile/Latex Safety Gloves
• Closed-toe shoes with socks
• Long pants
• Face Mask

• GUIDELINES FOR THE LENDING OF LABORATORY
  APPARATUS, GLASSWARE AND EQUIPMENT
• The Group Leader shall submit accomplished
  Borrowers Slip before the start of the
  experiment. The slip shall be signed by the group
  leader including members and be approved by the
  Laboratory Instructor.
• The Laboratory Technician shall prepare all the
  necessary apparatus, glassware, and equipment
  before the laboratory class.
• The borrower shall check all the issued items
  before signing the acceptance part of the slip
  and shall leave his/her ID card to the Laboratory
  Technician.
• The borrower shall return the items and it will
  be checked by the Laboratory Technician if they
  are complete and in good condition.

  APPARATUS, GLASSWARE AND EQUIPMENT
• If there are no losses or breakages/damages, the
  group shall be cleared, and the ID will be
  returned.
• If there are losses and breakages, all
  breakages/losses shall be charged on the account
  of the students concerned and the reference
  guidelines for losses and breakages shall be
  applied.
