2 Overview
Zipsurvey is online survey tool. It is being
provided by the Evaluation Accountability
Collaborative for Extension faculty who want to
use web surveys for conducting program
evaluations and as a learning tool for those who
want to learn about developing and implementing
web surveys.At this time it is a shared
resource (single license but multiple users).
There are few controls to prevent one user from
inadvertently modifying or deleting another
users work. Please be aware of these facts as
you work within Zipsurvey. Take care to ensure
your work does not affect another user. Read this
guide in its entirety before embarking on your
own project. Be sure to create and use your own
folder!We hope this tool works well for

Start a new survey.
Work with an existing survey. (main work area)
View results / export data to another format.
Create manage email lists.
Create email invitations or a link to put a
survey on a web site.

Start a new survey.
7New Survey
Most often youll select this option to create
a survey from scratch.
8New Survey
The new survey (with a generic name) now appears
in the list of all surveys. Rename it immediately
by clicking on Details.
IMPORTANT note that you are now working in the
My Surveys area.
IMPORTANT At this point, go ahead and create
your own folder or place this new survey in your
existing folder. Then come back to My Surveys and
select your folder (your new survey should be
there).See the Managing Folder Section near the
end of this Guide.

Note that everyones surveys are listed because
the Folder is set to All Surveys.
To specify basic survey options.
To delete a survey and all its components.
Not necessary but you can edit buttons and error
messages.
Set basics of the survey such as title,
background color, opening and closing
instructions, etc.
Add or edit questions to your survey
Select your own folder. Or click on Manage
Folders to create your own folder.
To make a copy of an existing survey.
Open / Close toggle. Change to Close once the
survey concludes.
11Survey Details under My Surveys
Enter a survey title. Use a descriptive title
that includes your county / unit name or number.
Copy and paste into this field the link to a logo
(use this link for the TCE logo).
Various display options. For background color,
beige is a nice alternative to white.
Opening page of the survey what appears before
the questions.

This is the one option you may want to enable.
Then click on Save.
13Survey Questions under My Surveys
Click on this to view a list of existing survey
questions (as shown below).
Click here to create a new survey question.
To skip to another question regardless of how the
respondent answer the current question.
To edit the question.
To skip to another question if a particular
response is selected. For example, if Q1yes then
go to Q3.
Change the position of this question in relation
to the other questions.
To delete a question.

Click on Help for an example of what each
question type looks like.
From the pull-down menu, select the question type
you want.
15Entering Information for a Question
Optional specify a more descriptive variable
name such as age, gender, etc. if you want.
Optional Enter any question instructions here.
Click this to make answering the question required
Enter the question here.
Enter each possible response (hit Return or Enter
after each). If you plan to export the data and
work with it in SPSS or Excel, youll want to
work with numbers instead of labels. In that
case, add value after each label so that a
number will appear in the database. For example,
Vernonvalue1, Fort Stocktonvalue2, etc.
will place 1 in the database instead of
Vernon, 2 instead of Fort Stockton, etc.
For Other response that can be typed in.
If you want this question to be the last one to
appear on the page. Click this for every question
if you want one question per page.
16Viewing / Testing the Survey
IMPORTANT! Just viewing the survey without
completing any questions does not count as a
completed survey. However, completing responses
(i.e. testing) does count as a completed survey
check My Account to see if the count is
decreasing.
Click to launch a survey (to view it or complete
a test survey).
After all testing is complete and youre ready to
launch the survey, you can clear the database
under View Results.

To delete an email list.
For the currently selected email list, add email
addresses individually here.
To create a new email list.
For the currently selected email list, delete
email addresses individually here.

1. Enter a name for the new email list.
2. Click on Submit to accept the name.

1. Paste your email addresses from another
application into this window.
2. Hit Submit to save.

Create email invitations or a link to put a
survey on a web site.
23Ways to Launch a Survey
There are four ways to launch a surveyA. Place
a link to the survey on a web page (does not
prevent multiple completions of the survey by
the same individual). Click on Create Links to
generate a link.B. Send an invitation to your
participants using your email software such as
Groupwise, Outlook, etc. with the link in the
invitation. Click on Create Links to
generate a link.C. Send an invitation to your
participants using Zipsurvey without unique keys
(does not prevent multiple completions of the
survey by the same individual). Click on
Email Invitations to create the invitation. The
link Survey Link will be automatically
included in the body of the invitation. D. Send
an invitation to your participants using
Zipsurvey with unique keys (does prevent
multiple completions of the survey by the same
individual). Click on Create Links to
generate the unique keys first. The link Survey
Link will be automatically included in the
body of the invitation. Then click on Email
invitations to create the invitations and enable
the unique keys.
24Launch Survey
Create unique keys (to prevent multiple responses
per individual) or to generate a link to place on
a web site.
Create email invitations.
25Creating an Email Invitation
Enter your email address.
Enter individual email address OR specify an
email list from the pull-down.
Enter a descriptive subject line.
Once you have created unique keys (under Launch
Survey), you can click on this to employ them
to prevent multiple responses per individual.
Enter the body of the invitation here built
around the SurveyLink.Do not alter the contents
of the SurveyLink at all although it can be
moved.

Creating Unique Keys
Use this if you are using email invitations
exclusively (not using a URL on a web site) and
you want to be sure that each participant
completes the survey once only.Enter a few more
keys than you have potential survey participants.
Simply post this link on your web page or include
it in an email to your participants (but without
unique keys it cannot limit one survey per
participant).

Unique keys that were created are displayed.

Be very careful that you clear YOUR results
only not someone elses.
To email a link to the results to others.

To view results as frequencies, percentages, and

Check number of free surveys remaining.

Number of free surveys left under the current
subscription.
If you are sending email invitations, enter your
email address here. ZipSurvey will notify you
when the invitations have been sent and which
invitations were undeliverable.
Do not change the account password!
32Manage Survey Folders
Start by clicking on Manage Folders under My
Surveys.
33Assigning a Survey to an Existing Folder
Select the folder you want the survey to belong
to (i.e., the folder you created for your
projects). Click on Update to save.
34Create a New Folder
Enter the name of the new folder (i.e., your
project or name). Click on Save (to keep) or
Cancel (to discard).
35Edit a Folder
Two edit are available1. Change a folder
name2. Delete a folder (any surveys
assigned to this folder are not deleted but
become unassigned).
Click Update to save edits or Cancel to
disregard.
