  • Improve the quality of life for individuals
    with HIV disease by responding to their existing
    and emerging needs and to provide educational and
    behavioral strategies to targeted populations, to
    reduce and prevent the spread of HIV/AIDS.

  • Roles and Responsibilities
  • Role of CFAP Consortium
  • The role of the CFAP Consortium shall be to
    provide a forum for the infected and affected
    members of the community, providers and other
    community members for the purpose of planning
    and prioritizing Care Act Title II and Florida
    General Revenue funds as well as prioritizing
    HIV/AIDS prevention needs by defining risk
    populations and specific strategies and
    interventions.
  • This role is an advisory capacity to the
    state.

    and grievance procedures

  • Membership
  • An active voting member is one who has
  • Completed a membership application,
    confidentiality statement and a conflict of
    interest form annually
  • Attend new member orientation
  • Attend two consecutive meetings
  • Participate in a standing or ad hoc committee

Article Four
  • Membership Terms are for one year but can be
    renewed as often as desired by member
  • Recruitment of members shall be from all service
    areas, infected and affected populations groups
    and various fields of expertise
  • Representation from all types of groups will be
    encouraged, anyone from business to religious
    groups etc.

  • Duties will include the participation in the
    planning, implementation and evaluation of the
    comprehensive plan, continuing education and
    providing information, referral, advocacy,
    support and education to others regarding HIV
  • Resignation can be given at any time by
    submitting a written resignation to either
    chairperson or the lead agency

  • Removal- Any member may be recommended for
    removal from the Consortium if it is determined
    by the majority of Executive Committee that the
    member in question does not have the best
    interest of individuals infected and affected
    with HIV while participating in Consortium
    business, and must be approved by a majority vote
    of the membership present at the Consortium
    meeting

Article Eight

  • Annual Meeting- February
  • Regular Meetings- Meet on a monthly basis, second
    Thursday of the month 1200, HFUW
  • Executive- Monthly
  • Committee- meet as needed
  • Special Meetings- may be called as needed
  • Quorum- simple majority vote of members must be
    at least ten
  • Voting/Proxies- each member has one vote, proxies
    may only given to PLWH/As

  • Voting
  • Each CFAP Consortium member shall have the
    right to attend and vote at any general or
    committee meeting and can vote on each issue with
    which they do not have a conflict of interest

Article Nine
  • Priority Setting and Resource Allocation
  • In order to vote during these meetings you must
    be a member in good standing (active voting
    membership requirements) and attend any Data
    Presentation.

Article Ten
  • Conflict of Interest
  • A conflict of interest occurs when any active
    voting Partnership member knowingly takes action
    intended to influence the conduct of the
    Partnership in such a way as to confer any
    financial benefit on the member, family
    member(s), or any organization in which he/she is
    an employee or has significant interest. A
    Partnership member who also serves as a director,
    trustee, a salaried employee, or otherwise
    materially benefits from association with any
    agency which might benefit from the priority
    recommendations of the Partnership is deemed to
    have an "interest" in said agency or agencies

  • This Consortium shall not participate or
    intervene in any political campaign on behalf of
    any candidate for public office
