Title: Overview of Hearing Aid Fitting and Verification.

1
Overview of Hearing Aid Fitting and Verification.
  • Dee Gamble
  • IHR Nottingham

2
Content of talk
  • Basic principles
  • Click Fit Vs Frequency Shaping
  • Individual examples

3
Three basic principles
  • Use generic prescriptions wherever possible, if
    not use manufacturers agreed target
  • Verify using REIG/REAG in all patients
  • Adjust programming of aid if there is a
difference between target and the measured
    response

4
Manufacturers agreed target
  • NAL NL1 for adults
  • DSLi/o or NAL-NL1 for children
  • The generic target needs to have been
    authorised to ensure appropriate modification.

5
Verification with REIG/REAG
  • If theres a target curve, based on mean data or
    KEMAR,
  • check it with REIG/REAG/RESR
  • at the time of first fitting adult/child
  • on all patients because we cannot predict whose
    ear acoustics differ from average.
  • Use the appropriate input signal (speech
    preferable)

6
Guideline
  • If its not
  • /- 5dB at frequencies 250 - 2k Hz
  • /- 8dB at 3k and 4k Hz
  • slope /- 5dB per octave
  • Investigate the reason for the difference
  • Adjust the programming if appropriate
  • Always use clinical judgement

7
REIG/REAG at follow up
  • Repeat REIG/REAG/RESR at later appointments if
    clinically indicated
  • Always listen to the patient/child/teacher/parent

8
Working protocol the ideals
  • Aids to be set up for everyday use
  • Compare REM H/A response with the REM target
within the software of the Aurical/Unity
  • Use equipment that allows two NOAH modules to
work simultaneously i.e. one for measurement and
the other for adjusting the hearing aid

Summary
  • DSP aids offer great advantages to our patients
  • Care is needed in following REIG/REAG protocols
with appropriate targets and input signals for
    each type of aid
  • Verification can help in maximising the advantage

12
  • All of these issues are covered in more depth
    during the practical training days.
