to work collaboratively toward the prevention of
homelessness and the creation of lasting
solutions for homeless and at risk families,
children and individuals
3PATH at the Coalition
A transitional program designed to 1. engage
individuals who are living on the streets or in
shelters many of whom are chronically homeless
2. help them obtain permanent housing 3. provide
the supports necessary for them to remain
successfully housed and stable within the
community 4. ultimately, link them with
long-term service agencies in the
community

5When I first got my apartment, it was great. I
was happy and everybody was happy for me. Then
after a while I realized livin inside on my own
was a lot harder than I thought. I was feelin
bad- real bad- all the time and I couldnt figure
out what was happening or what to do about it.
