- Required in the event of an accident or injury.

a copy of your last troop bank account statement.

Commitment to Support Girls in Troops
Each girl attending troop/group meetings, series,
camp, events, virtual must be registered. Troop
leaders or facilitators must have copy of health
history at each event or activity where the girl
is present. The Annual Parent Permission form
allows parents to complete one permission form
for activities throughout the membership
year. Gives permission for the Troop Leader or
First Aider to administer medications during Girl
Scout activities. A resource for you as you
build your Troop Committee and encourage
parent/guardian involvement.
8A New Troop Needs Start-Up Funds

Loan Agreement for New Troops This form is a
loan agreement between the troop and GSCO to
provide up to 50 start-up funds. Loan must be
re-paid by end of membership year.
9A Girl Needs Financial Assistance

Opportunity Grant Completed by the parent and
troop leader. Opportunity Grants are available
for uniform components, Journey books, and
assistance with activities and events.
10Talking to Girls about Sensitive Issues

Sensitive Issues Permission This form is
required and must be completed by troop leader
and signed by parents when discussing sensitive
issues with the girls. This form is available in
