Care pathways definition
  • An Integrated Care Pathway determines agreed,
    multidisciplinary practice based on guidelines
    and evidence where available, for a specific
    patient/client group. It forms all or part of
    the clinical record, documents the care given and
    facilitates the evaluation of outcomes for
    continuous quality improvement. 1

Aims of the Evidence Based Contents Service
  • To gather together clinical knowledge and
    organisational processes to develop care pathways
    .
  • To translate developed care pathways for Local
    Service Providers (LSPs) to deliver within the
    Care Record System (CRS) clinical applications.

What will the Evidence Based Contents Service
deliver?
  • A collation of all evidenced based knowledge from
    the current and future CfH Knowledge Process
    Safety projects.
  • National evidenced based care pathways from
    collated evidence.
  • An evaluated and validated care pathways using
    the integrated care pathways appraisal tool.
    (ICPAT)
  • Translated evidence based care pathways into
    SNOMED and data entry items.
  • Published and delivered care pathways to LSPs
    and LHC through the care pathways specialist
    library.
