Teacher Survey Guide Key Points
  • Should be completed only for students who have
    attended 30 days or more.
  • Should be completed by students academic day
    teacher for secondary level a Math or English
    teacher, preferably one who does not staff the
    21st CCLC.
  • Survey Guide contains a blank survey for you to
    tabulate returns.

Definitions
  • These terms frequently get confused with other
    common terminology (i.e. feeder school).
  • Please make sure you understand what it being
    asked!

Feeder School
  • Any public or private school that provides
    students to the
  • 21st CCLC.
  • This is entered in the
  • Grantee Profile.

Regular Attendee
  • Refers to students who have attended a 21st CCLC
    program for at least 30 days (which do not have
    to be consecutive) during the reporting period.

Reporting Period
  • The reporting period for the annual performance
    report coincides with the school year and
    includes the summer prior to the school year.
  • So for this report, summer 2007 and school year
    2007-08.

Subcontractor
  • An organization that is under contract with the
    grantee to provide 21st CCLC grant-funded
    activities or services. For APR purposes, a
    subcontractor is considered to be a type of
    partner.

Grantee Profile Check
  • Contact Information is current, both under both
  • 1. Basic Info, and
  • 2. Centers.
  • Partnerships are current.
  • Funding Sources are current.

Partners Key Points
  • Individuals can be considered partners.
  • District staff can be considered partners as long
    as the district is not the fiduciary (fiscal)
    agent for the 21st CCLC grant in question.
  • Districts can be considered partners if they are
    not the fiduciary (fiscal) agent for the 21st
    CCLC grant in question.

Estimated Monetary Contributions
  • We suggest using the amount that it would cost
    the program to attain those same resources, if
    they had to pay for them.  Usually for buildings,
    both schools and organizations will have a rental
    price.  Also many school districts and
    organizations have salary ranges for different
    level positions.  For volunteers/staff without
    specialized training we recommend using minimum
    wage multiplied by an estimate of hours of
    service provided.  We find that many schools and
    organizations who are contributing the services
    already keep track of these figures for tax
    purposes so asking someone in the accounting
    department for an estimate usually has good
    results.

StaffingKey Points
  • If an individual was paid to work at the Center
    for any percentage of their salary, they are
    considered paid staff.
  • Individuals who only worked at special,
    nonrecurring, or episodic events, field trips, or
    programming should not be counted in the staffing
    section.
  • On the line paid staff not funded directly by
    21st CCLC program, only count individuals whose
    salary was funded entirely by a non21st CCLC
    source.

  • Under Special Services or Programs
    Racial/Ethnic Groups, none of the information on
    these sections is optional. Please provide data
    on all of the sections.
  • For Limited English Proficiency, you can ask for
    students who were counted as 1, 2 or 3 in the
    October counts.
