Fill out the Profile for yourself as Principal
Investigator. If this is an NIH application, be
sure to include your NIH Commons User ID in the
block at the red arrow above. If there are other
faculty, enter their information as well.

Use are indicated so as to show the Assurance
numbers. Mark yes on yours only if they apply,

Related Project/Performance Site Location. Once
it is moved over to the right hand box, click
Open Form.

20The next mandatory form for this application is
the PHS 398 Cover Page Supplement. Move it to the
box on the right and click Open Form.

have the opportunity to attach PDF files that
address specific issues and aspects of your
proposal. Any topics that have no relevance
should be left blank.

which is required for your proposal. Well look

Modular Budget Pages show two periods (years) per
page. No budget breakdown is required on this
page.
32Budget Justifications allow you to tell the
Sponsor who will be actively working on the
project, and give a brief description of what
the main cost elements are that led you to choose
the funding module you did. As with all
attachments, use PDF files.
In our sample proposal budget we have only 2
years so, budget periods 3, 4 and 5 are left
blank. The cumulative budget information
populates automatically. Budget justifications
are highly recommended.
33Be sure to click on either Project or Subaward.
Start date for period is pre-populated. Fill in
the end date for the period.

34This 2nd page of the budget carries items like
Equipment and Travel.

(Year) and spot for up-load of Budget
Justification PDF. Note the Next Period button
in the upper right. Click it to generate the 2nd
Period (Year).
36Be sure to update the Start Date and End Date for
this 2nd Year
For our sample proposal, this 1st page of year 2
is the same as the 1st page of year 1.
37In this 2nd Year it is anticipated that there
will be less travel than in Year 1.
38This final page of year 2 shows the total for
that year to be 112,500. Scroll down to the next
page gives a cumulative budget.
39This cumulative budget of 262,500 should agree
with the amount on the cover page of the proposal.
40You are done! Save on your hard drive. E-mail the
proposal to your ORCA Administrator see the next
slide.
41The final part of the proposal is to complete the
BYU Internal Summary Sheet. This document will
carry the signatures of the Principal
Investigator, the Department Chair and Dean (or
Associate Dean). The Summary Sheet is then sent
to ORCA when the proposal is e-mailed
over. Allow 2 to 3 days for processing and
submission of your proposal.
