• Provide instruction
• on Forms 5 A-D

• Teachers complete point-in-timeForms 5 A-DScore
  Sheets
• Update Forms 2and 4, if needed
• Make photocopies of assessments toprepare
  forsubmission to AADSI (Alternate Assessment
  Data System, Internet) in Tucson Unified School
  District.

• Put photocopies of assessments in the mail to the
  address below
• April 8, 2005 is the due date
• April 11 through 29, 2005, AADSI will validate
  and input data into system
• AADSI will contact you for any necessary error
  correction

• AADSI will finalize data, create official reports
• Assessments and official reports will be sent to
  Luvette Russell, BIA, for distribution return to
  Agency schools
