  • What Do I do??

Initial Referral
  • The PPS team is called ONLY after the Student
    Care Team/TAT Team has met previously to do
    interventions on the student.

  • The Student Care Team/TAT Team
    should have met at least two times if not more
  • Identifies what data or information will be
    collected- Teacher may
  • come in with this ahead of time
  • Initial baseline information is collected and
    presented
  • Interventions are determined to assist the
    student during the 4 to 6 week period

Interventions VS Accommodations
  • Interventions should be specific to the concerns
    of the child.
  • Interventions are NOT moving the seat of the
    student, shortening assignments, allowing
    extra time for the test, test read, taking the
    test in a different setting.
  • Give the student 1-1 for flash cards, work on
    reading fluency with cbms and the speech
    teacher, have the LD teacher pull for small
    group in Wilson

  • PPS Team has reviewed the information shared from
    the Care Team/TAT Team
  • It is decided that a referral should be
    considered
  • Relevant information is discussed about the
    various domains
  • Parents can notified that a referral has been
    made and a meeting will be set to discuss
    concerns.
  • This should not be a surprise to the parent if
    the student went through care team/tat team.

Evaluation Time
  • The team has decided that a case study is
    appropriate.
  • Open up the evaluation section in
    myservicetracker
  • Information from the referral section will flow
    the section on the domain sheet
  • Information can be added or deleted by the tips
    for filling out the domains
  • IEP team members are notified at least 7 days
    prior to the actual domain meeting with the
    parents to provide the appropriate information as
    indicated by the tips for filling out domains
  • Testing decisions or data to be collected should
    be inputted prior to meeting with parents

  • Notification of Decision to Conduct
  • DO NOT wait until the day of the MDC to turn this

PPS Coordinator
  • Some sort of tracking system of the various
    components should be implemented with a check off
    sheet to indicate when ever person has completed
    the various parts of the case study.
  • ONLY after all parts are completed does the PPS
    coordinator contact the supervisor to arrange an
    MDC date unless otherwise arranged with the
    supervisor of your building.
  • Each tester is responsible for completing the
    summary of findings for the appropriate sections.

  • The academic performance section is about the
    only section that can be left blank in terms of
    progress in the classroom. If it is a middle
    school or high school student the guidance
    counselor should put in a summary of current and
    past grades/credits.

The PPS Coordinator can open an eligibility page
and an IEP.

Speech Articulation only
  • Speech therapist brings concerns to PPS
    coordinator.
  • PPS meeting is set up and formal written
    notification is given to Regular Ed. Staff,
    Parent, Speech Therapist, Nurse, Social Worker
    and Psychologist. Any other relevant personal as
    needed.
  • Information must be shared and/inputted prior to
    the meeting to the PPS Coordinator.
  • Psychologist, Social Workers and Nurse may input
    their information directly into the domain page
    or provide written documentation to the PPS
    coordinator. Provided this is communicated with
    the PPS coordinator prior to the meeting nurse,
    psychologist and social worker attendance is not
    necessary.

Speech- Language Issues

    coordinator.

    Parent, Speech Therapist, Nurse, Social Worker
    and Psychologist. Any other relevant personal as
    needed.
  • Information must be shared and/inputted prior to
    the meeting to the PPS Coordinator.
  • Psychologist, Social Workers and Nurse may input
    their information directly into the domain page
    or provide written documentation to the PPS
    coordinator. Provided this is communicated with
    the PPS coordinator prior to the meeting nurse,
    psychologist and social worker attendance is not
    necessary if and only if the domain is not
    relevant and requiring testing. E.G. The
    student is doing well academically. There is no
    concern at this time that intelligence or
    academic achievement is compromised. The student
    is functioning well socially and emotionally..
