    students.

OUR BIGGEST PROBLEM
  • DATES
  • Please make sure that signatures and dates are
    correct.
  • Students cannot begin experimentation before date
    of adult sponsors approval.
  • Most signatures must be dated before
    experimentation.
  • Exception is Form 1CRegistered Research
    Institutionwhich must be signed and dated after
    experimentation.

  • If possible, please have students complete (type)

  • It is an easy process and it makes the forms much
    easier to read.
  • Forms cannot be saved so students will have to
    have access to a printer.

SIGNATURES
  • All signatures should be signed in BLUE ink.
  • That way we will know which forms are originals
    and which are duplicates.
