- MFP is the single largest investment in Medicaid
Long Term Care - 31 States have been awarded 1.4 Billion with a
projected number of over 37,000 individuals to be
transitioned - Federal opportunity to further develop community
integration strategies, systems, and
infrastructure for people with long-term care
needs - Emphasizes community living vs. institutional
placement to help rebalance the system - A program that identifies individuals in
institutions who wish to move back into the
community and assists them with the transition
process
4MFP Demonstration Funding
- States receive an enhanced federal medical
assistance percentage (FMAP) for 12 months for
each Medicaid recipient who transitions from an
institutional setting into the community during
the demonstration period - Covers only expenditures on current and
demonstration home and community-based services
not acute care services - Virginias Rebalancing Amount - 38,508,185

- Four-year enhancement period, from July 1, 2008
through September 30, 2011

- Application developed in collaboration with the
Office of Community Integration for People with
Disabilities, numerous state agencies,
self-advocates, their families and other
stakeholders - Virginia was selected to participate in the
Demonstration in May 2007 - Launch date of July 1, 2008

- Eligibility Requirements Who Can Participate?
- Have lived for at least six (6) consecutive
months in a nursing facility, an intermediate
care facility for individuals with mental
retardation/intellectual disabilities, or a
long-stay hospital licensed in Virginia, which
can include periods of hospitalization - Be a resident of the Commonwealth of Virginia
- Be Medicaid eligible for at least one month at
the time of discharge - Qualify for PACE or one of the five following
waiver programs - Elderly or Disabled with Consumer-Direction
Waiver (EDCD) - Individual and Family Developmental Disabilities
Support Waiver (DD) - HIV/Aids Waiver (AIDS)
- Mental Retardation Waiver (MR)
- Technology Assisted Wavier (TECH) and
- Move to a qualified residence. A qualified
residence is 1) a home that you or your family
member owns or leases 2) an apartment with an
individual lease, with lockable access and
egress, that includes living, sleeping, bathing
and cooking areas over which you or your family
has domain and control or 3) a residence in a
community-based residential setting in which no
more than four (4) unrelated individuals reside.

- Permanent changes to our home and community-based
waivers - New Waiver Services
- Transition Services/Funding is a one-time,
life-time benefit assisting with one-time,
up-front household expenses. Added to EDCD,
AIDS, TECH, MR and DD waivers. - Time limited to 9 months (Up to 2 months prior to
discharge for MFP Participants only) - Transition Coordination supports participants who
elect services through the Elderly or Disabled
with Consumer Direction Waiver both before and
after transitioning to the community. - Time limited to 14 months (Up to 2 months prior
to discharge for MFP Participants only)
9Added Existing Waiver Services
- Assistive Technology - includes devices that
enhance ones ability to function and
communicate, such as specialized toilets,
braces, chairs, and computer hardware and
software. Added to EDCD and AIDS waivers. - Environmental Modifications - assistance with
making modifications to homes and primary
vehicles to make them accessible. Added to EDCD,
AIDS and TECH waivers. (For MFP Participants,
this service can be delivered prior to discharge) - Personal Emergency Response System and Personal
Emergency Response System medication monitoring.
Added to AIDS and TECH waivers.

- Emergency Regulations were approved and signed by
the Governor on June 24, 2008 - went into effect
on July 1, 2008 - Policy manuals affected have been updated to
reflect changes in services and are available on
the DMAS website - MFP Appendix has been added to the policy manuals
- Medicaid Memo sent to all HCBC Providers, MCOs,
AAAs, CSBs, Local DSS, and CILs on June 11, 2008
