What is Assistance?
  • In all DPI domains, assistance is defined as any
    employment-related assistance or intervention
    provided for the participant by your service or
    funded by your service
  • Evidence is, simply put, verification that the
    assistance will be, or has been, provided by your
    service

Expression of Interest Form
  • This documents the first contact a potential
    client has with the agency
  • It contains basic information on the client
    including their disability type and cultural and
    linguistic barriers
  • All contact with the client is documented in file
    notes on the rear of the form and constitutes
    evidence

Registration Interview
  • Registration interview is attended by two staff
    members
  • First staff member is responsible for general
    registration paperwork and procedures
  • Second staff member records observations and
    feedback from the client and their support network

Registration Interview
  • Evidence is recorded on the Registration DPI
    Evidence Report
  • This evidence shows the current interview skill
    level of the client and helps highlight areas you
    may need to work on with the client
  • Other evidence may be provided in the form of
    client (or support network) self reporting

Career Review
  • The Employer Consultant meets the client for a
    career review to ascertain their employment goals
    and complete the Activity Agreement
  • They document the career review in the clients
    file notes and use their observations to begin
    filling out the DPI Evidence Report
  • They continue filling out the evidence report
    over the four week period

Bringing it all together
  • Employer Consultants utilise the information from
    their file notes, the Registration DPI Evidence
    Report and their notes in the DPI Evidence Report
    from the Career Review
  • Once the information has been compiled and each
    section rated the Employer Consultant and I sit
    and process the DPI

Bringing it all together
  • Field staff focus on the positives of clients
    where DPI questionnaires are focused on potential
    barriers clients face in securing and maintaining
    work
  • Having a central point responsible for entering
    all DPI and DMI questionnaires into EA3000 allows
    for consistency in levels and funding of clients

Onsite Evidence Collection
  • Evidence is collected in a number of ways on the
    job site
  • Job Co-ordinators make direct observations of a
    clients performance and the support they have
    required
  • Feedback is sought from supervisors and
    co-workers
  • Worksite performance and issues are discussed
    with the client

  • Evidence can also be collected from a range of
    other valuable sources
  • Discussions with the clients support network
    should be documented and can provide valuable
    insight into how the client really is going
  • If you have referred a client on for more
    assistance then feedback from treating
    professionals, Occupational Therapists organising
    workplace modifications etc can also be very
    useful in completing your DMI

Onsite Evidence Collection
  • All of this evidence is recorded in the staff
    members PDA and later transferred into the
    clients online file notes
  • Feedback from staff regarding the use of PDAs and
    online file notes has been very positive

File Notes
  • Edge uses basic templates for the main file note
    types to
  • promote consistency in the information gathered
    by different staff
  • provide a structured format to ensure all
    relevant evidence is captured
  • assist staff with collating information from file
    notes into the Work Reports for completion of a
    DMI

Work Reports
  • The Work Report is completed using the evidence
    gained over a three month period
  • Each question is completed showing a brief,
    concise comment on the issue along with the
    frequency of support
  • As your evidence sources are shown on the Work
    Report, it is a quick and easy reference for
    completing the DMI

Bringing it all together
  • As with the DPI we start by asking the staff
    member to tell us about the client and their
    performance in their own words
  • We review the number of hours of support the
    client has received in the preceding months
  • We discuss what level of funding the
    co-ordinator feels would be appropriate

DMI Collaboration
  • We then proceed to process the DMI in EA3000
  • As with the DPI, the Work Report is a working
    document until the DMI has been authorised
  • When the DMI is completed, the online scoring
    should match the Work Report

  • These are very simple and easy to implement
    systems for gathering evidence
  • At an agency and individual staff level these
    systems have worked very well for Edge and have
    been fairly painless to implement
