- Clear Biological Cal Test from CCA-200
3DD 2216 Periodic Hearing Test Requirements
- A Valid DD 2215 is Needed to Perform a DD 2216
- The DD 2215 May be in the DOEHRS-HC Database or
- Obtained from the Patient Record
- If a Valid DD 2215 Cannot be Found in the
Database or - Patient Record, a DD 2215 Must be Performed

4Steps for Completing a DD 2216
- Prepare Patient for Testing (Includes Obtaining
Their Demographic Data) - Enter the Patients IDN in DOEHRS-HC Transfer
It to the CCA-200 - Start the Test in the CCA-200
- Enter/Update Patient Demographic, Test Type HPD
Data - Monitor the Test Progress in the CCA-200
- Retrieve the Results from the CCA-200
- Add Remarks, if Applicable
- Print the Test and Print the Referral, if
Applicable - Clear Test Results from DOEHRS-HC and the CCA-200
5Enter Patients IDN in DOEHRS-HC and Transfer It
to the CCA-200
6Return to DOEHRS-HC and Enter/Update Demographic,
Test Type and HPD Data If a DD 2216 Periodic Test
is Selected and Reference Thresholds do Not Exist
for the Individual, the User is Prompted to
Enter the Reference Thresholds and Test Date
7Reference Thresholds and Test Date have been
Entered, Click Save
8Reference Data Successfully Saved Message
NOTE Manually Entered Reference Thresholds DO
NOT Provide Enough Information to Produce a DD
2215 Hearing Test, but are Adequate for
Completing a DD 2216 Hearing Test
9Continue Completing Audiogram Type Information
NOTE 14 Noise-Free Hours are NOT Required to
Complete a DD 2216 Periodic Hearing Test
10If Yes to HPD Used, the HPD Selection Window
Opens Select HPD Used, Including Double/Secondary
Protection, Click OK
11When the Hearing Test is Finished, Retrieve the
Results from the CCA-200
12DOEHRS-HC Checks
After DOEHRS-HC Retrieves Test Results from the
CCA-200Automatic Checks for Referrals and/or
Reports are Performed Based on the Type of Test
Completed
- DD 2216, Audiogram Checks
- Is a Positive or Negative STS Present?
- (/-10 dB or Greater Average Change at 2k, 3k,
AND 4k Hz Or - /-15 dB or Greater Change at a Single
Frequency 1k, 2k, 3k OR 4k Hz) - Is an OSHA Reportable Hearing Loss Present?
- (25 dB or Greater Average Change at 2k, 3k,
AND 4k Hz, in Either Ear, as - Compared to Original Baseline. Only Reported
First Time Occurs Per Ear and - When Additional 25 dB or Greater Decrease
Occurs Per Ear) - Is Masking Required?
- Is an Asymmetrical Hearing Loss Present?
13Earliest Reference Thresholds are Needed to
Calculate OSHA Reportable Hearing Loss If there
is an Earlier Reference Test in the Medical
Record Click Yes
14DOEHRS-HC will Prompt You to Enter the Earliest
Reference Test Date and Thresholds
15You will be Prompted to Verify that You Want to
Save the Thresholds Entered After Clicking Yes,
You will be Returned to the Run Test Window

Remarks on the DD 2216
24Print the Test, Using One of the Applicable
Buttons or the File Menu List
25If Print Preview is Turned On, the Test Appears
on the Screen
26Click the Printer Button and then Click OK to
Print the Test
27Complete Two DD 2216 Hearing Tests
- A 90-day Hearing Test for the Person You Tested
Yesterday (DD 2215 in the Database) - An Annual Test Using a NEW Listener (DD 2215
NOT in the Database)
