• 6 hours maximum within a 24 hour period use
  your beginning time as the start of 24 hour
  period

• Use this form for o/t worked during patrol
  enforcement and traffic safety checkpoints.
• The blanks in top right section are for totals of
  all contacts/charges for each page
• Grant - upper right hand corner of agreement

• Ensure logs have printed name and signature of
  both officer and supervisor
• An officer cannot sign as his own supervisor

• If multiple pages for 1 shift indicate page 1
  of 2, 2 of 2 etc.
