What is The First Report?

• The first report of injury tells Risk Management
  Safety everything we need to know about the
  injured employee, and the employers impression
  of the an injury that occurred in course and
  scope of employment

What does RMS Accept?

What is the Wage Statement?

• The wage statement tells RMS the injured
  employees gross wages earned 13 weeks prior to
  the date of injury, and allows RMS to accurately
  calculate the compensation rate.

What is the Supplemental Report of Injury?

• The Supplement Report of Injury lets RMS know
  about subsequent loss time not reported on the
  First Report of Injury.

What is the Request for Paid Leave?

• The request for paid leave tells RMS when the
  injured employee has elected to use accrued sick
  or annual leave.

• You can down load these forms to your computer,
  and electronically submit to RMS via an e-mail.
• First Report to
• WCIFIRSTREPORT_at_TAMU.EDU
• All other forms will go to the applicable
  adjuster.
