WHAT DID WE DO?
  • PICKS UP INFORMATION FROM GP DATABASE (EMIS), AND
    DEPOSITS IT INTO A CONTACT ASSESSMENT

Development of the Mail merge Contact Assessment
  • The Contact Assessment is the basis of the Word
    document
  • Each field in the Word document relates to an
    EMIS code - taken from clinical templates
  • Most of the codes are standard EMIS ones for
    the ones which arent, we have a list
  • GP already held much of it

  • Another surgery has started by asking patients to
    complete Basic Information, inputting it into
    EMIS, and then setting
  • up an Integrated Team

  • GP has full picture of patients wishes, needs
    and points of contact.

    involved service can share the Overview
    Assessment, Care Plan and goals

    user-friendly
  • Closer working with colleagues
  • Receiving feedback on referral very helpful
  • Reduction in requests for additional information
    (because all provided on the Contact Assessment)
  • Database more uptodate
