  • Incident reports for four nurses in our hospital
  • Compared to the National incident reports
  • Variance is quite significant

What We Discovered
  • 79 errors were made over a period of 2,598 shifts
    worked in the local hospital
  • This is an error rate of 3.04 compared to the
    national rate of 0.96
  • Three times the error
  • rate is a cause for
  • concern!

Local VS. National Data
  • National data suggests that a higher number of
    errors occur on the third shift, thus prompting a
    constraint to add more nurses for that shift
  • On the other hand, local data suggests that 2nd
    shift has the most errors. Therefore, it is
    suggested that additional nurses be scheduled on
    2nd shift instead of 3rd shift for this
    particular hospital

  • National data suggests more nurses are needed
    for Wednesday and Saturday
  • However, we recommend that additional nurses be
    scheduled on Saturdays and Sundays due to the
    difference in the local data

  • After further testing of local data we discovered
    that there is no correlation between years of
    experience and the number of incidents reported
  • Therefore, on the local level it is not necessary
    to pair less experienced nurses with more
    experienced nurses because it will not reduce the
    incident rate
