Health insurance information

• You may buy health insurance from UAF
• A much cheaper alternative is available from the
  company United Healthcare

• The plan number is 001716-01
• The name of the plan is New York Service center
  for Chinese Study Fellows
• You must have a credit card to pay for it

Bank money transfer information

• If you or your parents do not have a credit card,
  they can transfer money to UAF using a bank
  transfer
• The UAF account number is 2680-032-6.
• The web address to inform UAF about the transfer
  and what it is for is http//www.uaf.edu/finsvcs/f
  orms/Wire_IN.pdf.
• Please email me (john.lehman_at_uaf.edu) how much
  you have sent and what it is for so that my staff
  can make sure it goes in the correct account

Turning in your application before I leave Beijing

• I will be in my office (204 International center)
  Friday from 930 until 500 except during
  lunchtime.
• I will be best to turn in your applications on
  Friday
• Saturday I will be in Changping, but not in the
  office most of the day. Please call 1343 6676
  476 if you want to turn in your materials and I
  will arrange to meet you.

What to do if your application is not ready by
Saturday April 26

• If it is mostly ready, I can take it back and you
  can give the rest of the material to Professor
  Du.
• Otherwise, please give the application to
  Professor Du.
