What If I Need Care Away from
Home?

• EMERGENCY Go to the nearest appropriate medical
  facility
• Notify your PCM as soon as possible to ensure
  payment of claim
• NON-EMERGENCY Care Covered only when you
  receive prior authorization

How do I make an Appointment?

• Start by calling 1-800-931-9501.
• Select Option 1 for either a Primary or
  Specialty appointment.

• If youre unable to secure an Urgent Care
  appointment through the TSC, you always have the
  option to speak with your Primary Care Manager
  IF YOURE ENROLLED IN PRIME!! If youre not sure
  of the number, ask when you call the 1-800
  number.
