Introduction
  • If youre investigating fertility problems and
    looking into IVF as a treatment, you may have
    heard of hysteroscopy.
  • Hysteroscopy is a procedure that allows your
    doctor to look inside your uterus in order to
    diagnose or inspect for any abnormality.
  • Hysteroscopy is done using a hysteroscope, a
    thin, lighted tube that is inserted into the
    vagina to examine the cervix and inside of the
    uterus
  • Since hysteroscopy examines the lining and
    interior of the uterus, Hysteroscopy may be
    recommended as one step in the evaluation of a
    number of gynecological problems, including
  • Abnormal vaginal bleeding
  • Retained placenta or products of conception after
    a birth or miscarriage
  • Congenital (inborn) anatomical abnormalities of
    the female genital tract
  • Polyps or fibroid tumors inside the cervical
    canal or inside the uterine cavity

Hysteroscopy Procedure
  • What procedures can a gynecologist perform
    with a hysteroscope?
  • After a general anesthesia is given the
    hysteroscope is inserted into the uterus using a
    salt solution (NACL) or a sugar solution
    (Sorbitol) to distend the uterus and obtain
    visualization of the uterine cavity.
  • After completing the inspection of the uterine
    cavity several different instruments may be
    inserted through the hysteroscope to help treat
    uterine fibroids, heavy menstrual bleeding and
    polyps.
  • The surgeon also attempts to visualize the
    openings to the fallopian tubes (tubal ostia).

    remove these non-cancerous growths found in the
    uterus.

    Syndrome, uterine adhesions are bands of scar

  • Role of hysteroscopy in recurrent implantation
    failure (RIF)
  • Recurrent Implantation failures means three times
    failure of good quality embryo to implant onto
    the uterine wall during IVF treatment.
  • Implantation failure may be due to embryonic
    factor or uterine factor. The quality of embryo
    can be accessed microscopically and with the help
    of preimplantation genetic screening, but its
    difficult to evaluate uterine receptivity. Minor
    pathologies if missed on sonography(TVS) which
    can be the cause of IVF failure can be diagnosed
    and treated on hysteroscopy.

What is IVF? (In Vitro Fertilization)
  • Eggs and sperm are taken and fertilized outside
    the body in a laboratory to produce embryo which
    is then transferred into the uterus by the
    infertility specialist.
  • This transferred embryo should get attached to
    the inner lining of the uterus (endometrium) and
    start growing (Implantation)and develop into a
    successful pregnancy.
  • The most important success determining step of
    IVF is Implantation which in turn depends on
    endometrium and its receptivity.

Role Of Hysteroscopy In IVF
  • Unsuccessful IVF attempts usually are due to
    factors in the embryo, such as genetic problems,
    or issues with the woman's uterus.
  • In the past many fertility clinics routinely
    performed hysteroscopy on women who had failed
    IVF cycles, to look for uterine growths or scar
    tissue and remove them
  • Hysteroscopy Is It Essential Before IVF?
  • There is no role of routine diagnostic
    hysteroscopy for evaluation of uterine cavity
    before IVF in each and every case.
  • With the advent of very good trans-vaginal
    sonography (TVS) and sono-salphigography it is
    not warranted to do diagnostic hysteroscopy in
    all IVF patients
  • only in those patients with abnormal findings on
    trans-vaginal sonography or abnormal
    sono-salphingogram can be treated with
    therapeutic hysteroscopy.

What Are The Benefits Of Hysteroscopy?
  • Hysteroscopy is a good diagnostic and therapeutic
    tool in IVF.
  • In case of recurrent implantation failure and
    recurrent abortions it is helpful in correction
    of pathologies and thus improving pregnancy
    outcome and would be helpful in saving the cost
    of patient who are undergoing repeated IVF cycle,
    where failure occur due to missed pathologies.

How Safe Is Hysteroscopy?
  • Hysteroscopy is a relatively safe
    procedure. However, Just like any medical
    procedure, you could have some complications from
    a hysteroscopy, including
  • Problems from the anesthesia
  • Infection
  • Tearing or damage to your cervix, though this is
    rare
  • Problems with gas or fluid from the uterus
  • Damage to nearby organs like the bladder, bowel,
    or ovaries
  • Pelvic inflammatory disease
  • Reaction to the substance used to expand the
    uterus.
  • If you experience symptoms like a fever, severe
    abdominal pain, or heavy bleeding after the
    procedure, call your doctor immediately
