• In Fatal Facts Accident Report No. 21, OSHA
  describes an accident in which a chicken ladder
  broke and two workers fell 35 ft. from the roof
  of a barn. One worker sustained multiple
  injuries and the other worker was killed.

• Equal in length to the width of board
