    requirements including evaluation
    responsibilities

    HSAT Action Plan in the areas of NE and FS

  • one foodservice action step
  • one nutrition education action step
  • Paper copy is due December 15, 2006.
  • After you are notified that it has been approved
    you must enter this information on your HSAT
    on-line Action Plan.

Evaluation Requirements
  • Facilitator Updated HSAT Action Plan by
    12-15-06, End of Year Questions, Final HSAT
    Update (end of project).
  • Lead Teacher/Educator student and teacher
    evaluation for NE implementation
  • Foodservice Director required FS eval forms
