What is it all about?

• Virtual Multi-Disciplinary team
• combining professionals from health, social
  services and the voluntary sector
• meets every six weeks to discuss complex
  neurological cases in the community
• to coordinate service input, promote joint
  working and improve communication
• lack of resources and/or information in the
  Borough are highlighted

Complex Neurological cases

• A need for coordinating different services
• Early referral or referring difficulties
• Difficulty working with carers
• Cognitive and/or insight difficulties
• Denial
• Rapidly deteriorating condition
• Advice on self funders
• Clarifying previous service involvement
  changing roles
• Exploration of joint funding
