2
The Evolution of Booking
  • Waiting times
  • Choice of provider
  • Choice of appointment
  • NHS modernisation agenda

3
The way we were
  • Patient sees GP
  • GP refers to consultant
  • Hospital sends appointment to patient
  • Long Waits no negotiation of appointment

4
Evolution to include more patient choice
  • Partial booking process for all MPCT outpatient
    clinics introduced 2002
  • Centralisation of MPCT outpatient booking 2003
  • Introduction of private provider capacity for
    Orthopaedics (July 03)
  • Choice for patients on surgical waiting lists
(Since Jan 2001 over 1700 patients accepted
choice offer and moved for shorter wait)

5
Benefits from Choice
  • Smaller waiting lists from over 2,500 patients
    to 1,450 for elective surgery
  • Same is true for outpatients
  • Shorter waiting times. From 15 months and 26
weeks to 6 months and 13 weeks

6
Lessons learnt
  • Patients more likely to attend if appointments
    are negotiated
  • Clinics more effectively utilised if booking
    centralised
  • Offering choice for patients already on surgical
waiting lists has 2 main drawbacks
  • 2 outpatient consultations needed
  • Lack of clarity if consultants at 2 providers
    dont agree

7
Choice at Point of Referral
  • More efficient
  • Only one outpatient appointment
  • Clarity of consultant opinion
  • Patient negotiated time/place of appointment
  • Choose and Book system designed to streamline
    process

8
Choose and Book System
  • Patient sees GP
  • GP decides to refer to consultant
  • GP considers and discusses with patient options
    of where is clinically appropriate
  • Patient contacts Booking Management Service to
discuss selected options, considering wait times,
    location/logistics
  • Patient agrees convenient appointment

9
Department of Health monitoring of Choose and Book
  • Strategic Health Authority ranking
  • Primary Care Trust ranking
  • GP sign up to discussing choice with patients
  • Targets set for practices and PCTs
