PROCEDURES

• New procedures and forms put into place in late
  2006 replace all previous instructions related to
  direct care shared staff
• Procedures and forms can be used by group homes,
  apartments or in any living situation where
  shared staff are used to provide direct care

IMPORTANT.

• If transportation is included as a service, you
  must explain how transportation will be used.
• Staff fringe benefits are not required to be
  justified if 25 or less however, if they are
  over 25, the fringe calculation worksheet must
  be submitted.

Fringe benefits cannot exceed 32

  each person is displayed in the Cost per Person
  cell

To Complete Budget Spreadsheet.

• The Cost per Person figures are transferred into
  the Shared Staff line of the Budget spreadsheet
• in the annual salary, fringe amount and total
  salary cells.
• All other information in the Budget Spread
• sheet is entered based on each persons
• one on one needs.

REMEMBER.

• If a person is living in the community, a group
  home or apartment and is receiving only supported
  living services, they can continue using the
  limited moderate model at 900 month with the 15
  day minimum of services or the limited minimum
  model at 400 month with the minimum 10 days of
  service

PLEASE NOTE.

• If a person lives in a group home, apartment, or
  community and is receiving other services besides
  supportive living or their cost (based on
  individual need) exceeds the 900/400 level, a
  MAPS meeting must be conducted to determine the
  new level of services needed
• MAPS narrative must explain what occurred that
  caused person to need services above 900/400
  level.
