- You incur a medical/dependent care expense
- Complete a FSA claim form and attach receipts or
an Explanation of Benefits (EOB) - Mail to ASI
- ASI reimburses you directly from your spending
account - ASI can also auto deposit money into your account
and notify you by e-mail the date and the amount
that was deposited

lifetime monthly benefit. - Vesting occurs after 5 years with 100 of
employer contributions available after 10 years.
