The Overall Goal
  • Good quality clinical data
  • All facilities capturing the data in the same
    consistent manner.
  • Improved communication between facilities and the
    Ministry.

Data Quality is a collaborative approach
  • Data quality needs to be a the front end of the
    data collection process with good communication
    to the front line staff
  • Involves multiple stakeholders, hospitals,
    Ministry, ICES.
  • Reporting of problematic areas to facilities
  • Ongoing education to provide better information
    to hospitals to ensure consistent data

CIHI Coding Standards
  • The CIHI Coding Standards are the benchmarks
    which the Ministry monitors the Acute and
    Ambulatory Care data.
  • Managers need to ensure coding staff have the
    time to review and be familiar with the Coding
    Standards.
  • The CIHI Coding Standards and the Management
    Information Standards have been incorporated into
    the Hospital Annual Planning Submission.

Data Quality Indicators
  • The data quality indicators developed by the
    Ministry the majority are based on the CIHI
    Coding Standards.
  • Some are reviews in basic concepts for good
    quality data such as Patient age with Patient
    Service 54 cannot be greater than 1 day.
  • Main Diagnosis of Arthritic Knee with a knee
    procedure
  • Main Diagnosis of Arthritic Hip with a hip
    procedure.
  • Principle procedures cannot be coded as
    Abandoned.
  • Coding of Chronic Obstructive Lung Disease with
    Pneumonia
  • Incorrect coding of Obstetrical interventions

Data Quality Indicators
  • Previous Caesarean Sections incorrectly captured
  • Incorrect coding of endoscopic converted to open
    procedures
  • Coding of Gastroenteritis versus Non-Infectious
    gastroenteritis
  • Coding of IV insertions

Mandatory Data Requirements
  • CAT scans and MRI are mandatory on both DAD and
    Ambulatory Care Data - Project 260 or Project
    265
  • Diabetes on Acute and Ambulatory Care (regardless
    of treatment)
  • Anti-thrombotics which are administered need to
    be captured whether the condition is Myocardial
    Infarct or Deep Vein Thrombosis.
  • When coding Z51 within Ambulatory Care data a
    Cancer diagnosis must be captured as the Other
    Problem. Interventions need to be captured to
    insure correct grouping assignment.

Mandatory Requirements
  • Project 100 - Inpatient, Day Surgery and
    Emergency
  • At the time of coding must indicate whether or
    not the chart was complete to code.
  • If the chart is incomplete, then indicate the
    chart is incomplete
  • Should the chart be reviewed after completion
    then the field for date of review must be
    completed.

Ongoing Initiatives
  • The DQ indicators from 2004-05 and 2005-06 have
    been sent to the University of Waterloo to be
    incorporated in the WHIC Tool for 2006.
  • There will be new indicators developed for
    2006-07 as CIHI incorporates some of the previous
    indicators into their edits.
  • The five Data Management Coordinators will now be
    responsible for Data Quality for Acute and
    Ambulatory Care Data.

  • Ensure your Clinical Volumes for Inpatient and
    Ambulatory Care match the volumes CIHI is
    reporting to the Ministry for your facility.
  • Verify your Clinical Volumes match your MIS
    volumes for Inpatients and Ambulatory Care.
  • Inpatient
  • Emergency
  • Day Surgery
  • Oncology
  • Dialysis
  • Cardiac

  • 2005-06 Q4 - Year End Timeline
  • May 31st, 2006
  • Information submitted to CIHI following this date
    will not be forwarded to the Ministry.
  • 2006-07 Timelines can be found posted on the FIMB
    website under Announcements 2005/06.
