- Vitality encourages members with incentives and
rewards for healthy choices - The Vitality Program focuses on four key areas

activities and advance Vitality Status along the

6Vitality Check
- Entails a physical examination for height, weight
and blood pressure and a blood draw to measure
cholesterol, blood glucose and tobacco use. - Employer site screening available with our
partners - LabOne, Life Time and other health
club partners. - Vitality Checks are offered for all DHP at Life
Time clubs on specific dates and times. Schedule
and online registration available on the website.
Benefits of offering worksite screening is better
participation and employees not required to take
½ day to get blood work done.
7Vitality Check through LabOne
- Champs contact LabOne directly at 800-654-7824.
- Employer and LabOne will sign agreement and
schedule screening. - A minimum of 20 participants will be required for
onsite screening. Cost is 43.50 per
participant. - Once employer agreement in place, employees will
have access to select ExamOne walk-in facilities
- cost is 88.50 - Services include physician oversight of the
screening and monitoring of results as required
by each state. Eliminates responsibility and
liability on employer.
8Vitality Check Sample Results
- Members will receive their results in
approximately 2 weeks.
9Vitality Check at Life Time Fitness
- A schedule of Vitality Checks available at Life
Time Clubs will be available this week. Members
can register online via our website for the
location, day and timeslot. - The cost is 70.00 per person. Member will be
charged at time of screening. Cash, check or
major credit card are accepted. - Worksite screenings are also available through
Life Time Fitness for employers located near any
LTF facility. - A minimum of 30 participants is required for LTF
worksite screenings.
10Vitality Check Other Provider
- Members who cannot obtain a Vitality Check
through one of our partners can submit results
from physician exam or from other wellness
provider using Vitality Check form available on
website. - Bucks will be awarded for results actually
received.Members will also receive the stand 750
Bucks obtaining Vitality Check.

month for previous months activity - Once data received, Bucks will automatically
appear in members Vitality Profile
13Where to Obtain Fitness Assessments
- Performed by Life Time Fitness and other Vitality
Health Club Partners - Free to all members of Life Time Fitness
- Destination Vitality newsletter contains coupon
for 25 for non-LTF members. Coupon is necessary
as it includes day pass for club and is required
for obtaining fitness assessment. - Members who do not live within 15 miles of a
health club partner can complete a qualified
fitness assessment at a facility of their choice. - Fitness assessment forms available on website
- Certified trainer to complete documentation
Qualified assessments are those that include
body composition, cardiovascular testing with a
sub max VO2 score, strength and flexibility
testing.

Available once per lifetime.
- Individuals must be of The Destiny Health Plan
and their health club for at least 4 months and
provide proof of club membership. - The weight reduction program rebate is available
to those with a starting BMI level of 30 or
greater.

- Maximum amounts that can be applied to your
monthly PMF Booster in interest and your PMF
Carryover Booster are 1,500 for single
coverage, 2,625 for you and one dependent, and
3,750 for family coverage. Earned booster
monies are taxable to you. - The life insurance multiplier applies to the
first 100,000 of life coverage. - The critical disability coverage multiplier
applies to only critical disability claims,
regardless of whether or not your Guardian Life
Insurance Company Disability contract includes a
Critical Disability Rider. Critical Disability

33Vitality Program Rules
- Vitality Status is determined by Bucks earned in
a policy year. Status is not determined by Bucks
used or remaining balance. - Vitality Bucks accumulate in a members Vitality
Profile until they decide to spend them. - Spendable Bucks and earned Vitality Status
carryover at plan year renewal , however members
start over in terms of earning their Vitality
Status for the next plan year. - Bucks expire two years after the end of the
program year in which they are earned. Oldest
Bucks are automatically spent first. - Note Destiny Health reserves the right to change
the program at any time.
