2The Partnership Model is a model of care delivery
that is founded on the belief that to deliver
effective, high quality care a series of
partnerships need to be in place.

7Create a vision of excellence in patient care
delivery
8Implement a comprehensive assessment of care
delivery examine in detail the care and service
delivered by the nurse and other providers.
9Create a design team to review/examine role of
CNL, RN and other care givers.
10Reorganize work responsibilities around CNL and
non-RN care providers to ensure each role is
functioning at its highest level.
11Create clear and specific job descriptions with
identified functions and responsibilities of each
role.
12Steering Committee should review comprehensive
assessment to identify barriers to work flow and
system failures and assign rapid cycle
performance improvement teams to lead initiatives
to streamline processes to support patient care.
13Identify specific outcome measures that are
important for evaluation of pilot. Obtain
baseline data before pilot, i.e. LOS recidivism,
complications, patient satisfaction, staff
satisfaction. Consider using data sets that
already exist in your institution. Plan for time
line of data review.
14Identify pilot units. Consider the units that
have strong leadership to manage the pilot
through the change process.
15Communicate the concept and intent of the pilot
to physicians, clinical and non-clinical
stakeholders.
16Educate the staff on role changes. Provide them
education as necessary to strengthen staff
competencies of communication, delegation,
assessment, data analysis, evidence-based
practice, etc.
