  than September 15 for application deadlines prior
  to November 1 and by November 15 for Regular
  round applications
• Form found on-line on ASP website under Upper
  School/College Guidance

Transcript Request Form

• Transcript Request Form must be completed for
  universities outside of UCAS or the US Common
  Application
• Indicate mailing address or online submission
  link ( 3 address labels if mailing)
• Submit to the Guidance Office by Sept 15
  applications due prior to November 1 and by Nov
  15 for later deadlines.
• Attach university forms for counselor to
  complete

Teacher Recommendation

• Request must be made by Sept 15 for deadlines
  prior to November 1 and by Nov 15 for later
  deadlines.
• Most institutions require no more than two
  teacher recommendations
• Select the same teachers for all recommendations
• Attach university forms for teacher to complete
