Account Assignment Path 5A
Account assignment form requested by the IPM once
the CLEC/Reseller starts the contract process
with CenturyLink. When Account Assignment form is
completed it is sent to the appropriate contact
for assignment. 5B

• CLEC/Reseller may choose to
• Negotiate (If parties do not agree with
  contract language)
• Re-negotiations (expired agreement)
• Opt In in its entirety (negotiation not
  required) (If Parties agree to contract
