17Guidance notes for helping someone fill out this
booklet Please note this booklet is not a person
centred plan, it is however a tool, designed to
help gather information as part of the planning
process. ( a little plan in the
making!) Hopefully it will get people thinking
about their lives and what they want for
themselves. It may also bring to light any issues
in their lives that people feel they want to look
at in more depth, thus flagging up the need for
more planning to be done with that person if this
is their wish. The booklet can be completed in
key work times, people can work on them
individually, with one to ones or as a group,
staff could also complete their own booklets to
use as examples? The booklets are for people to
complete in anyway they so wish, this could
involve photos, magazine pictures, symbols or
drawings. The emphasis is on making it individual
to that person, using forms of communication
that are appropriate to them. I appreciate that
we are all busy and this is yet another thing we
have to do!!! However you might even find it
useful and dare I say it an enjoyable experience?
And just think, when it comes to reviews youll
have gathered a lot of the information already!
18The relationship circle This could be filled
with either names or photo, it could even contain
Pop stars, T.V characters or people who have
passed away.Dreams This can be absolutely
anything, from getting married to flying a plane.
The importance is in the fact that we all have
and need dreams, this gives us something to aim
for, I might never be able to fly a plane, but I
could maybe have a go in a flight simulator or
have a look around a cockpit?NightmaresThis
could be something as simple as being afraid of
spiders to feeling lonely or having a fear of
dying. If we talk about the things that frighten
us then we can begin to understand them and cope
with them. Please feel free to add your own
sections to this booklet, maybe the person would
like a section about how they communicate? Any
qualifications or work experience they might
