Process Map for Providing Dog Licenses at Rabies
Vaccination Clinics Current map as of September
7, 2012
Resident or non-resident dog or cat owner comes
to rabies vaccination registration table
If documentation is not available or
incomplete ie New pet (initial
vaccination -unknown vaccination history -forgot
to bring documentation of previous vaccination
Health Dept staff requests/reviews proof of
previous rabies vaccination, if available from
pet owner
If documentation is available and complete
Pet owner leaves and returns with required info
during clinic hours
Pet owner is provided with a list of future
rabies clinics in the area that they may
attended ie after pet owner is able to obtain
required paper work for a 3 year booster
Health Dept staff completes the rabies
vaccination form
Animal may receive a one year booster(animal must
be at least 3 months old however)
Pet is vaccinated by vet
Pet owner and animal move to vaccination area
Pet owner and pet leave thru exit door
A copy of the completed form is given to the pet
owner by Health Dept staff
DOG LICENSE OPTION Colts Neck dog owner comes to
dog registration table for license (pet owner
may be a walk-in who did not attend this rabies
clinic, as well)
Health department staff determine if this is a
renewal or first time dog license

Health Department staff looks up dog license
record for the pet on computer database or via
hard copy of records
Health Department staff requests owner and pet
information as well as required paper work ie
Rabies vaccination certificate Spay/neuter
verification of discounted license
Any necessary updated information is given to
Health Department staff, if required. ie updated
rabies vaccination certificate or equivalent
Dog owner is able to obtain required information
during clinic hours

Pet owner is provided with information by Health
Department staff on how to obtain license at town
hall or mail in application for license
