FARA LA. Driver Safety Program
  • The Louisiana Drivers Safety Program is a
    comprehensive part of the FARA (F.A. Richard
    Associates, Inc) formally LA. ORM (Office of Risk
    Management). Each state agency must identify
    which employees will be authorized to drive on
    state business.

FARA LA. Driver Safety Program
  • Any employee that drives on state business must
    complete the Authorization Driving History
    Form, provide a copy of their drivers license and
    complete the Defensive Driving course offered
    online by the
  • Education Department. Authorized drivers must
    provide proof of a current drivers license every
    year upon request from the Driver Safety
    Coordinator.

Moving Violations
  • Any authorized driver that receives a moving
    violation in a state vehicle or private vehicle
    must report ALL moving violations to their
    supervisor and the Driver Safety Coordinator
    within 24 hours. This includes moving violations
    received in your personal vehicle whether you are
    on state time or your personal time. Any
    violations not reported to your supervisor and
    the Driver Safety Coordinator may result in loss
    of driving privileges and/or disciplinary action.
    Upon reporting a moving violation you will be
    required to complete online within 14 days.

High Risk Drivers
  • High risk drivers are those who have 3 or more
    convictions, guilty pleas, /or nolo contendere
    for moving violations within the previous 12
    months.
  • Having a single conviction, guilty plea, or
    nolo plea for operating a vehicle while
    intoxicated, hit run driving, vehicular
    negligent injury, reckless operation of a
    vehicle, or similar violation within the previous
    12 month period.

Unauthorized Drivers
  • If an employee is NOT authorized to drive, the
    employee their supervisor shall be notified in
    writing that the employee is not authorized
    shall not drive a state or private vehicle on
    state time. The supervisor the employee
    must sign the Not Authorized to Drive form
    return the form to the Driver Safety Coordinator.
  • The supervisor shall assume responsibility for
    not allowing an unauthorized driver to drive a
    state or private vehicle on state time.

Reporting Accidents
  • All accidents shall be reported to the employees
    supervisor the Driver Safety Coordinator by the
    driver of the state or private vehicle on the day
    of the accident. If the driver is unable to
    complete the Louisiana State Drivers Accident
    Report Form (DA 2041), then the drivers
    supervisor will complete the form to the best of
    their ability. The DA 2041 form the police
    report must be completed within 48 hours
    forwarded to the FARA Claims Unit.

Supervisors Responsibilities
  • Supervisors are responsible for notifying the
    Driver Safety Coordinator of ALL moving
    violations revocation of employees drivers
    license. Failure to report may result in
    disciplinary action up to including termination
    of employment.
  • Supervisors shall ensure that
  • employees complete the mandatory
  • Defensive Driving course within 30
  • days of employment
  • every year thereafter.

Supervisors Responsibilities
  • Supervisors shall allow only authorized drivers
    to drive for state business on state time.
  • Supervisors shall ensure that ALL accidents
    incidents are properly reported said records
  • are maintained.

Employees Responsibilities
  • Employees shall only operate vehicles for which
    they are licensed and insured.
  • Employees who are authorized to drive state
    vehicles are responsible for the safe operation
    of those vehicles.
  • Employees shall report any unsafe condition or
    accident involving a state vehicle to their
    supervisor or department director.

Employees Responsibilities
  • Employees shall immediately report any revocation
    of their drivers license or any moving violation
    received to their supervisor, but no later than
    their next scheduled workday. Said reporting
    applies whether on state or
  • personal/private business whether in a state
    or personal/private vehicle. Failure to report
    may result in disciplinary action up to
    including
  • termination of employment.
