  • MHM is paid on a reimbursement basis
  • Do not spend a disproportionate amount of
    Commission funds for speaker(s)
  • Out-of-state travel costs are not permitted or
    reimbursable
  • Reimbursement for health screenings will be on a
    per unit (per-person) cost basis

  • Personal Service Contracts for the events should
    be specific, in writing, and must be signed by
    each party
  • Self-purchasing is disallowed space, books,
    pamphlets, printing, staff etc
  • Up to 10 of the budgeted amount for each event
    can be used for healthy refreshments

  • Physician time for medical examinations is a
    disallowed cost
  • Utilize the mammography guidelines (available at
    the Commission)
  • Rental fees should be inclusive of all related
    costs for example janitorial, security, etc.

Final Report
  • A final report is due 15 days after the last
    event and no later than May 15, 2007
  • Final report located on the Commission website
    www.mih.ohio.gov
  • Submit an itemized expense report
  • Only those items listed on the approved budget
    will be reimbursed

  • Retain and submit copies of supporting
    documentation for Commission related
    expenditures i.e. receipts, invoices, etc.
  • Failure to submit a final report will result in
    non-reimbursement
  • Retain all supporting documentation until you
    have received an approved final report.

Local MHM Kickoff
  • Available Funds
  • Up to 5,000 for each of the major cities Akron,
    Cleveland, Cincinnati, Columbus, Dayton, Toledo,
    Youngstown
  • Up to 2,000 for the smaller cities
  • There must be a minimum of two funded agencies
    within each city to qualify for local kickoffs
  • History of Local Kickoffs in 2006

  • Inform community of upcoming activities
  • Promote minority health programs in the community
  • Demonstrate community collaboration and
    cooperation
  • Not for promotion of lead agency

Eligibility Criteria
  • Must be a Commission grantee
  • Mandatory 30 day Local Calendar of Events
  • Ability to deposit and track funds in a separate
    account
  • Due date to submit the lead Agency Application is
    December 11, 2006

    Proposed Activity and Budget
  • Coordination of local event between April 1st and
    April 7th
  • Establish and convene local kickoff meetings
  • Be in contact with funded minority health month
    grantees and others for Local Calendar
  • of Events and participation in kickoff

Local MHM Kickoff Lead Agency Responsibilities
  • Engage with media/public relations
  • Invite local and /or state elected officials to
    participate at the kickoff
  • Invite local Commissioner (s)
  • Pay approved expenses in a timely manner

  • All Commission funded projects are required to
    participate in their citys local kickoff
  • For local kickoff ideas, refer to the Lead Agency
    Application
  • Submit an itemized Final Report with supporting
    documentation (receipts, bills, invoices, etc.)
    to the Commission by no later than May 15, 2007
