    information from hospital system
  • Mostly data from lab tests
  • Questionnaires still in paper form and in Epic
    for Pulmonary Clinic

    assist in recruitment/follow-up process
  • Eliminate need for shared Excel spreadsheet
  • CRC can keep record of what data was collected
    from patient on each visit using interface
  • Assist with post-clinic workflow
  • Assist in determining who needs resampling of DNA
    or recapture of form data
  • No need to manually look for each follow-up in
    spreadsheet

  • Plan to develop into web-based interface

    to allow searching of DNA samples/genotypes by
    outcome, such as Diagnosis, test result, etc.
