- July 3, 1956 ---- National Health Survey Act
authorizing a continuous survey to provide
statistical data on the amount, distribution, and
effects of illness and disability in the U.S. - Established a Health Interview Survey with
periodic special surveys based on health
examinations (the Health Examination Survey) - Also proposed health records survey
