Client Number Amalgamation

• Amalgamating client numbers will leave a single
  client number in place of the ones that were
  amalgamated.
• The remaining client number will have the same
  name, address, and method of delivery information
  associated with it as it did before the
  amalgamation.
• Download the Client Number Amalgamation
  application from the ISC web site.

Client Number Amalgamation

• At the bottom of the Client Number Amalgamation
  form is an Affidavit of Identity.
• The client in Section A must enter his/her name
  on the Name line and place the name of the city
  and province or state on the Location line.
• The above named must sign on the Signature line.
• A Commissioner for Oaths, Notary, or Solicitor,
  must sign on the appropriate line.
