• CS/HB 537
• Provides that an absentee ballot request is
  effective for all elections through the next two
  general elections after the request is made,
  rather than just for the elections in the
  calendar year in which the request was made.

• After such a request expires the Supervisor of
  Elections will send a reminder to the voter to
  renew the request if desired.

• Creating an Absentee Ballot Request Form
• First Check with your local SOE for required
  information and whether they can supply a
  standardized form to you. If you must design
  your own form it must include at least the
  following
• The name of the voter for whom the ballot
• is requested
• The voters address
• The voters date of birth
• The requesters name
• The requesters address
• The requesters drivers license number, if
  available
• The requesters relationship to the voter and,
• The requesters signature (written request only).
  
• Make sure your SOE approves of your form before
  you use it or print it!
