• Photocopy of Passport
• Photocopy of Visa Information (if applicable)
• Social Security Number or ITIN
• If none, the visitor must complete IRS form W-7
• IRS Form 8233
• If all of these steps are not followed or if the
  person comes from a country where there is no tax
  treaty with the US, BYU must withhold 30 in
  federal taxes from the payment.
