- Open Enrollment Period
- February 25th to March 16th (midnight)
- Benefits Enrollment Hotline 381-6180 (7am-5 pm,
M-F) - Elections made during the Open Enrollment will be
effective April 1, 2008! - On-Line Open Enrollment Process!
- All benefits eligible employees must enroll even
if you elect not to participate or you have no
changes to your current election! - Benefits eligible Part Level II Level III and
Full-time - If an employee does not enroll during Open
Enrollment and currently has group health plan
coverage they will be defaulted to the following
plans - Catastrophic Medical and Incentive Dental

- Annual Requirements
- Personal Wellness Profile (PWP) complete online
from home or work, link provided at Inside St.
Lukes (http//www.stlukesonline.org/pwp/) - Health Screening schedule appointments through
St. Lukes Learning Center - Both requirements must be completed by June 30,
2008! - In 2007, 19 of employees were identified with
possible impaired glucose metabolism (FBG gt 100)
3 with FBG gt 120 were triaged by an RN
resulting in 4 new cases of type 2 diabetes - 2007 Results
- 87 met both requirements (2741)
- 13 did not meet one or both requirements (430)
- 312 appeals of those who appealed all but 15
were approved - Those who did not meet requirements and did not
receive approval via the Appeals Committee will
have an option to elect from the Catastrophic
Plan or Choice Plan

82008/2009 Plan Alignment Items
- Annual Open Enrollment will be offered every year
going forward - PTO Grant Program Magic Valley to implement
program in 2008 - Basic Life AD D Insurance 100 employer
paid benefit. Coverage aligned across the St.
Lukes Health System. Move to new vendor ING - Short Term Disability (STD) New name for the
SLHS will be Extended Sick Leave or ESL. - Magic Valley to pick up 8 hour annual bonus if at
520 hr max. - Magic Valley to adopt three tier accrual rate
based on yrs of service - SL (Boise/MMC/WR/MSTI) to sunset the 25 payout
for staff hired after 4/1/08. Staff who were
hired prior to 4/1/08 who terminate with 10 years
svc will receive the 25 payout of their STD
balance. - Vision Plan will be unbundled from Dental giving
staff more flexibility in electing coverage. New
Options - Incentive Dental

- NEW Bariatric Coverage Applies to major
medical (deductibles co-insurance apply),
lifetime maximum of 10,000 (including
complications). Note Must meet plan defined
requirements - NEW Pre-Diabetic Screening/Education 50
coverage after deductible has been met with a
maximum of 2 sessions covered - NEW Limit on High Cost Drugs 100 member
co-pay for drugs costing 1,000 or more for a 30
day supply. - NEW Topical Fluoride Therapy Treatment
following periodontal surgery - NEW Sealants No age limit on sealants (once a
3 year period per posterior teeth)

- NEW Voluntary Benefit Long-Term Care offered by
Med America - Subject to full Underwriting
- Customizable plan along with three tiered plan
options - Portable, you may take it with you should you
leave St. Lukes - Cash Plan for maximum flexibility this cash
plan may be accessed for expenses related to
homecare, assisted living as well as long term
care facility fees should the insured be eligible
to receive benefits. - Benefit may be purchased for yourself, your
spouse or your loved ones - Did you know.that more than 50 of the U.S.
population will require some type of Long-term
Care during their life time and that Long-term
care is not covered by Medicare or private
insurance. Medicaid is an option, but only if an
individual reaches poverty level, having spent
down all personal assets.

- Flexible Spending Account (FSA) Maximum for
Medical Spending Account increased from 3,500 to
5,000 - Note Total of Medical and Continual
Reimbursement may not exceed the new 5,000
maximum - Adoption Reimbursement Benefit extended to
adoption of step children, grandchildren and
other related children previously excluded - MSTI Fruitland Staff will now have the option to
elect the EPO Plan along with previously offered
Choice and Catastrophic Plans - Vision Plan Benefit for exam increased to 60,
bifocal and trifocal lenses increased as well

142008/2009 Premiums
- Premium Changes Premiums will increase on
average by 6 - Note Premium increases influenced by plan
enhancements, utilization, inflation and overall
increase in benefit costs. Also increase may
vary from plan to plan, average 6 overall - Preferred Premium Pricing for non-tobacco users
vs. Non-Preferred Pricing for Tobacco
Users. - Effective April 1,2008, tobacco and non-tobacco
users will have separate sets of medical
premiums. As announced last year, we will be
providing a preferred pricing to our non-tobacco
users. This new provision has been put in place
to support our mission of Improving the health
of people in our region and our tobacco free
campus initiative. - Along with this change, tobacco cessation
prescriptions drugs will now be covered under the
pharmacy benefit.
