check the status of authorizations and claims,
see the claims history of you and your covered
dependents, and review the covered benefits on

PM CST Monday through Friday.Â
3About Your Medical Benefits
EPO Plan - This plan offers in and out-of-network
benefits utilizing the HNP network. However,
out-of-network claims outside the coverage area
will be paid as in-network in the event of an
emergency. PPO Plan - This plan offers in and
out-of-network benefits utilizing the PHCS
network. PHCS is a national network that offers
additional flexibility at an additional cost to
the employee.

- EPO Plan
- HNP Network providers must be used by all
residents of the 11-County HNP Service Area (see
map of the 11 counties). This includes all
employees who work within the 11-County HNP
Service Area. All plan members who reside
outside the 11 counties must use providers in the
HNP Network when visiting the 11 counties except
during emergency situations. To find an HNP

PPO Plan The PHCS Network is a national

7Prescription Drug Information
- The prescription drug plan has a three-tier
co-payment structure with a formulary. The three
tiers are described in detail below. - First Tier Generic Generic drugs have the
same active chemical composition, the same
potency and can be offered in the same form as
their brand-equivalents. - Second Tier Preferred Brand Preferred-brand
drugs are those which generally have no generic
equivalent. - Third Tier Non-Preferred Brand Non-preferred
brand drugs are those which generally have
equally effective and less costly generic
equivalents and/or one or more preferred-brand
options.
