12
Action Team Members
  • Priority populations who are most affected by the
    issue.
  • Other stakeholders - agencies and groups affected
    by or addressing the issue.
  • Movers and shakers that can get things done in
    the community
  • People with time to work on action plans, skills
you need, and/or passion for the issues.
  • People involved in Community Health Assessment
on the CHA team, listening session hosts or
participants, or people who attended a community
forum where results were presented. These folks
already have a vested interest in the issues.

13
Determining Specific Objectives
  • Identify root causes of issue (Drill down)
  • Identify what's going on to address the issue -
    existing programs, services
  • Identify gaps in existing services
  • Discuss who is affected worst. What population
should be targeted? How many can you target and
    have an impact?
  • Brainstorm what can be done what changes do you
want to make in the most-affected populations?
    How could you do that?
  • Pray for consensus in the group

14
Determining Specific Objectives
  • Is there a good chance of success (it is
    politically possible)
  • Can you get the needed resources
  • Will the intervention benefit from a
collaborative approach involving a variety of
    community stakeholders
  • Is there a balance of interventions for short
term wins and long term lasting change,
  • Are the interventions multi-level and
    synergistic with existing activities.
  • How do we measure our success/how do we evaluate?
