Information.

• We initially found out about the upcoming Item
  number through the medical journals.
• We contacted the Division to get more
  information, so that we could prepare as much as
  possible before November 1st.

What we did

• 1. Decided as a practice whether we wanted to
  provide this service, i.e. capacity (appointments
  available), viability, value to the patient and
  treating GP.
• 2. When the decision was made, we discussed what
  we wanted the health check to include and how we
  could model it to be both viable for our practice
  and valuable to the patient.

Next steps..

• We worked on a model to address risk factors for
  chronic health conditions.
• When the model was eventually decided upon, we
  also decided how much scheduled time would be
  required to implement that model.
• When the scheduled time required was decided, we
  checked that it would still be viable to provide
  this service.
• We also checked our process against the Item
  Number Descriptor as soon as it was available.

Who is eligible?

• We pulled reports of all patients on our database
  who were eligible.
• The lists were given to the GPs for a final
  decision on whom to recall.
• A letter was sent to those patients identified,
  inviting them to attend.

Communication

• All GPs and staff were advised of the new Item
  Number, what the model and process was for
  completing these and how these appointments
  needed to be scheduled.

Review Process

• We will review the system by pulling reports on
  how many have been done we want to know
• 1. Have they been of value to the patient and
  GP?
• 2. Have our resources coped with the extra
  workload?
• 3. Have they been viable?
